Notice of Privacy


This notice describes how health information about you (as a patient of this practice) may be used and disclosed, and how you can get access to your health information. This is required by the Privacy Regulations created as a result of the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

Our practice is dedicated to maintaining the privacy of your health information. We are required by law to maintain the confidentiality of your health information. We realize that these laws are complicated, but we must provide you with the following important information: Use and disclosure of your health information in certain special circumstances.

The following circumstances may require us to use or disclose your health information:

  • To public health authorities and health oversight agencies that are authorized by law to collect information.
  • Lawsuits and similar proceedings in response to a court or administrative order.
  • If required to do so by a law enforcement official.
  • When necessary to reduce or prevent a serious threat to your health and safety or the health and safety of another individual or the public. We will only make disclosures to a person or organization able to help prevent the threat.
  • If you are a member of U.S. or foreign military forces (including veterans) and if required by the appropriate authorities.
  • To federal officials for intelligence and national security activities authorized by law.
  • To correctional institutions or law enforcement officials if you are an inmate or under the custody of a law enforcement official.
  • For Workers Compensation and similar programs.

Regarding your Health Information

You have the right to the following items regarding your health information:

  • You can request that our practice communicate with you about your health and related issues in a particular manner or at a certain location. For instance, you may ask that we contact you at home, rather than work. We will accommodate reasonable requests.
  • You can request a restriction in our use or disclosure of your health information for treatment, payment, or health care operations. Additionally, you have the right to request that we restrict our disclosure of your health information to only certain individuals involved in your care or the payment for your care, such as family members and friends. We are not required to agree to your request; however, if we do agree, we are bound by our agreement except when otherwise required by law, in emergencies, or when the information is necessary to treat you.
  • You have the right to inspect and obtain a copy of the health information that may be used to make decisions about you, including patient medical records and billing records, but not including psychotherapy notes. You must submit your request in writing to Great Basin Orthopaedics, 9480 Double Diamond Pkwy, Reno, NV 89521.
  • You may ask us to amend your health information if you believe it is incorrect or incomplete, and as long as the information is kept by or for our practice. To request an amendment, your request must be made in writing and submitted to Great Basin Orthopaedics, 9480 Double Diamond Pkwy, Reno, NV 89521. You must provide us with a reason that supports your request for amendment.

SMS Terms of Service

Company Name: Great Basin Orthopaedics

Purpose: This SMS Privacy Policy outlines how Great Basin Orthopaedics uses, protects, and manages the personal information of patients when communicating via text messages (SMS). The goal is to ensure compliance with privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA), and to protect the confidentiality and security of patients' information.

Consent:

  • By providing your mobile phone number and agreeing to receive text messages on your new patient paperwork, you consent to receive appointment reminders, health-related information, and other relevant notifications from Great Basin Orthopaedics via SMS.
  • You may opt-out of receiving text messages at any time by replying with the word "STOP" or by contacting our office directly at info@greatbasinortho.com.

Types of Information Sent Via SMS:

Great Basin Orthopaedics may use SMS to send:

  • Appointment reminders and confirmations
  • General health tips and wellness information
  • Notifications about office closures or other important updates
  • Requests to contact the office for more detailed information

Important: Text messages will not include detailed medical information, diagnoses, or any sensitive personal health information. All sensitive communications will be conducted through secure channels, such as patient portals or encrypted emails.

Data Security:

  • Great Basin Orthopaedics uses a HIPAA-compliant messaging platform to ensure that your information is protected during transmission.
  • While Great Basin Orthopaedics takes steps to secure SMS communications, it is important to note that standard SMS is not fully secure. By consenting to receive SMS, you acknowledge and accept the risks associated with this form of communication.

Opt-Out and Management of SMS Preferences:

  • You have the right to opt-out of receiving SMS messages at any time. To opt-out, reply with "STOP" to any text message you receive from us. Alternatively, you may contact our office by emailing us at info@greatbasinortho.com to request that we discontinue SMS communications.
  • Once you opt-out, you will receive a confirmation message, and we will cease sending SMS messages to your mobile number, except for messages confirming your opt-out request.

Third-Party Service Providers: Great Basin Orthopaedics may use third-party service providers to send SMS messages. These providers are contractually obligated to protect your information and comply with all relevant privacy laws, including HIPAA.

Patient Responsibilities:

  • Keep your mobile device secure and ensure that others do not have unauthorized access to your text messages.
  • Notify Great Basin Orthopaedics immediately if you change your mobile number or if your device is lost or stolen.

Changes to the Policy: Great Basin Orthopaedics reserves the right to modify this SMS Privacy Policy at any time. Any changes will be communicated to patients via SMS or through our official communication channels. Your continued use of SMS services after such changes signifies your acceptance of the updated policy.
